We require a consult request form for every phone appointment unless the pet has been seen at Tufts within the past 12 months for the same condition. This form should be filled out by a veterinarian (or employee of the veterinarian) who is actively managing the pet’s current health concerns. This veterinarian will serve as our main point of contact before and after the appointment.
This form should NOT be filled out by the pet owner.

Because of differences in available commercial diets in different countries, we are unable to do commercial diet consults for veterinarians or clients outside of the United States.
Simple home-cooked diets utilizing common ingredients used in the United States (chicken, beef, pork, fish, egg, barley, potato, corn, oats, etc) and Balance IT supplements (www.balanceit.com) can be requested by international veterinarians for their clients. However, we may not be able to identify appropriate alternatives if you are unable to find the specific ingredients locally or if the pet does not tolerate the supplement. Balance IT supplements will ship to most countries.
We need the last 6 months (or as appropriate) of medical records. For sick pets, a longer time period may be helpful. Please contact us if you have any questions about what to include or for exceptions.
